Supported Independent Living or SIL is a crucial support for those seeking to live in their homes, offering assistance or supervision with daily tasks, personal care, and meal preparation. Tailored to individuals with higher support needs requiring significant help 24/7, it aims to foster independence and skill development.

Aligning with the NDIS criteria, SIL may include:

  • Help with personal care tasks
  • Help to build your skills in things like meal preparation and cooking, cleaning, and developing a routine
  • Help to action any behavior support plans you have
  • Help to develop your social skills
  • Support with supervision, personal safety and security
  • Support to give you your medication
  • Support for medical appointments
  • Community access that is not routine or regular, for example banking, recreation tasks, shopping, spending time in a cafe.
  • Support to get to and from community access activities as per preference.

Empowering independence and enhancing quality of life, Hillberry Care proudly supports the NDIS (National Disability Insurance Scheme), connecting people with disabilities to support in their local community and provide funding for eligible people with disabilities.

You will be able to access the NDIS if you meet all of the eligibility criteria:

The NDIS is only available to people with disability under the age of 65.

02

You must be an Australian citizen, permanent resident or hold a protected special category visa to be allowed access to NDIS support.

03

You qualify under NDIS early intervention requirements if you are a child up to the age of six with a developmental delay that can be reduced by early intervention support.

Delivering care to address identified clinical necessities

Periodically reassessing requirements and revising the care plan accordingly

04

Collaborating with other healthcare providers when deemed necessary.

The DVA finances accredited Community Nursing (CN) providers to administer services to eligible DVA Veteran Card holders.

To receive community nursing services for an assessed clinical need, a client must have either a:

Veteran Gold Card, or

02

Veteran White Card

Please take note:

If a client is a Veteran White Card holder, the DVA will provide funding for CN services if the required care is attributed to an accepted service-related injury or condition.
